If I were to start the definition, I'll leave it blank and you fill it in for me. Male impotency is male. Impotency, actually, is the inability to obtain and maintain an erection for successful sexual intercourse. Is it basically an issue of the message coming from the head to the penis, or is it much more involved than that? Well, actually, it's much more involved than that. That is a critical point in which the signal from the brain to the penis is important. But also physical factors are also a problem and can be one of the leading causes for impotence. You know, it's a topic that some folks don't want to face. Some folks don't want to deal with it seriously. Who does it affect? Well, it affects the male as well as me. And I think there are a lot of psychological problems or psychological interplay between the partner and his mate and can actually be a reason for a lot of problems between the two. But is there a particular definition of the work definition? But the example of who is typically suffering from this type of ailment, are there a particular age group? Is there a particular cultural group that suffers from this this problem? Well, impotence can affect men of all ages. Males between 25 and 6575 can be affected by this problem. So it doesn't really affect any particular age group, nor does it affect any particular racial background. It affects all males. Perhaps some some patients feel that sex is dirty, so therefore they can't get erections. Some patients want to punish their their wives, so they become impotent, essentially things of this nature. Uh, and for these patients, we seek the advice of a psychiatrist prior to trying to do something about their impotence. Some cases we don't do anything because we feel that the the prostheses that we would use for their implements would not be effective. Other patients who have problems, which are emotional or psychiatric, we would go ahead and do something about it because they would be willing to have sex. What about the the physical physiological causes? The physiological causes are mechanical. They are related to blood vessel or blood supply to the penis or nerve supply to the penis. The patient might have a blockage, such as arterial sclerosis. The patient might have a spinal cord injury, like jumping into an empty swimming pool or diving into an empty swimming pool, cutting off the nerve supply from the brain down to the penis. Things like diabetes, which affect both the blood vessels and the nerves, affects the nerves going through the small blood vessels, and it affects the blood vessels also which go to the penis so that these patients don't get erections under the broad definition of sexual dysfunction. Is there any percentage idea you can give us in terms of among males who are suffering some form of impotency? Is there any percentage you can give us in terms of how many are having physiological problems compared to psychological? Surely about 35% of our patients are diabetics. About 24% are patients who have diseases which which affect the other pelvic organs. No. This, uh, situation that we do for patients is essentially not a new situation. Uh, male since has been treated, uh, since the 67 with, uh, press. Static replacement. So this is not really a new area that we're looking at here. In addition, in addition, the first recorded prosthesis was done by the Chinese at 1000 B.C.. My heavens, We should learn more about that. They're both a practicing urologists who have formed a group that more or less specializes in dealing with this problem. Tell us a little bit about treatment for impotency in terms of, you know, the prosthetic devices and so forth. Well, first of all, I think we should look at how impotence is classified. As we spoke earlier, there are essentially two classifications. Those that have physical or organic causes versus those who have psychogenic impotence. And what type of workup do you do to make that decision? Which sort of preliminary checkup over to have you? Do you do you decide whether or not you're going to need a prosthetic device? Well, the first initial evaluation would be a history, which is very important. That is followed by physical examination after these two things have been evaluated. We also evaluate patients with overnight tests. One of these overnight tests is called nocturnal penile tumescent monitoring. This is a test where we'll spring ages or place around the penis and they measure two methods of the penis during the sleep phase. It is known that every night, every 90 minutes, a patient will have an involuntary erection. And this is what we measured during that sleep phase. For example, if a patient is found to have erections during the sleep phase, then we can consider this to be psychogenic impotence. Now, you know you have a device in your hands. What is this and how do you utilize that in terms of diagnosis and treatment? This is one of the spring gauges that are that is placed around the penis during the sleep phase. Yes. You'll have to hold it. In other words, this is looped over the penis. And of course, this goes to a tumescent monitor machine that the patient would take home and the patient would use this during the sleep phase for two consecutive nights. And there is a chart or something that where the impulses are register. Yes, there is a chart that's recorded in this device. The patient would bring this device back to the office. Not immediately. With patients who have problems with psychiatric problems, you can probably detect something. But most often you can't. It would be better or behoove you to go through all the steps to find out specifically what is wrong with the patient, simply because once you do the procedure or a procedure on the patient, for all intents and purposes, he won't be able to get an erection as such. After that. Okay, now you've determined the patient's problem and you begin to describe something to help out. What kinds of devices or approaches or operations are then available. I'd like to just bring this in also. This is a simplified version of this. There are two strips. There are two strips, one is red and the other is blue. And this is placed around the penis. And as Dr. Hall said, during sleep, you have erections. If this is broken, then we say that the patient has had an erection and therefore he may be a candidate for surgery. And if it doesn't, then we say it's psychological. But as I mentioned earlier, even psychological cases, some deserve surgery now. But can a person who can a man who is impotent, have erections that would register even though they are impotent? Surely at during the day, are they smaller now? While during the day he may not have any at all? You say. Or when he desires to. When he desires to. So this is good? Up to a point. Then you must use your clinical judgment to decide whether the patient should have surgery. Some patients require psychological evaluation prior to having a prosthesis inserted. And we try to also bring the partner into this because, of course, the prosthesis is going to benefit both. Mm hmm. We do have a couple of, I believe, former patients in the studio today. By their request, they are appearing in silhouette and we have given them fictitious names to protect their identities, But they have agreed to discuss their situations with us. We'll be calling them Jack and Bill. Jack on your left and Bill on your right. Let me ask you gentlemen, am I correct in assuming that both of you have been through the procedure? Yes, that's okay. What can you tell us about before and after? Well. It's a normal thing if you want to do it. Uh huh. Yeah. You're brand new after 2 to 6 weeks. What do you mean you're brand new? And just a normal thing. Just like he was when he was 21 or 25. Let me ask you, Jack, what entered into your decision to have the operation? Because it was not working. That was not working. It was. Wasn't it working? Well, I can't answer the reason why it wouldn't work, but it just wouldn't work, and I couldn't do anything with it. Okay. Okay. And was that creating some problems for you? Yes. It was emotionally not so emotional. But, you know, when you have a family, you know, you're supposed to do certain things. How was your mate responding to your inability to watch any reaction? That was the only solution for my condition to be able to. Okay, We're good. Okay. Let me ask you, how long ago did you have the procedure, Bill? Well, I had that. How long have I had this procedure? About five years. And no problems? No problems whatsoever. Successful erections in the five years since? Yes. How about you? I might have been born a year. Okay. But I have no problems since I had the operation. The Bougainville's a success. Yes. Do. What type of success rate is there? Doctors. Dr. Saunders? Is it fairly reliable? Well, I think it depends upon what mode of treatment you use for the patient. If you're going to use a prosthesis, doing the prosthesis has a very high success rate. Okay. Of course, there are other modalities of treatment as well. Okay. Let's start right there so that we don't have to cut off. Let's come back to that and discuss some of those other procedures. Let's look at the ones we have here on the table that you brought today, and we can go through them in terms of success ratio and some of the things that you have to say about them. Well, essentially, there are four classes of prosthetic devices. Those can be broken down into inflatable prosthetics, prosthetic devices, which is what we have here with this one. Yes. Where does the the the bulb go in the body and where does this go? Which is to be popped so that you can get to it, to pop it. The reservoir is usually placed under the belly button. Okay. The pump is placed in the scrotum. Okay. And the two cylinders are placed in the penis already. Do you have you utilized this device in your own practices? Yes. Yes, we do. What about the success ratio? This has a rather good success ratio, but it's a mechanical device. And with all mechanical devices, there are rates of failure just based on mechanical defects. This prosthetic has approximately a 5% mechanical failure. And when there is a mechanical failure, that means a re operation. Mm hmm. So other prosthetics can be used. Let's look at some of the ones that you feel are a little bit more successful. I believe you call this one a malleable. Um. Yes. This is a malleable prosthesis. It's coated on the outside with silicone rubber. It has a braided silver wire within it. And the silver wire can be bent to create a flaccid versus erect penis. And so this one remains in the penis. Yes, it does. Okay. Now, let's look at this one that's on the chart or the board or whatever it might be term. This has just come out for the second time and it may become the cash, but this has just come out. And there were some problems with it early, but now it seems to work very fine. We spoke earlier about the Jones prosthesis, and I think this is probably the most commonly prosthetic device that's implanted and it's implanted because of its simplicity. Usually premature ejaculation is not considered a form of impotence In order to prematurely ejaculate, one has an erection. So this is not really impotence per se. Okay. We have some more issues that we'd like to air before this discussion is over. And we'll do that right after this break. We're back live and talking with doctors Roy Hall and Edward Saunders about male impotency. And it's a topic that is very emotional and one that obviously has a great deal of interest. Right now, we've got a question from our studio audience. My question is this is a relationship is based on love aren't in these basic principles. We learn as we grow up. And just because a man's impotent, does that mean the relationship cannot work without that feature of having sex? Doctor's Hall or Sanders. I think at the same time, obviously those factors are important and those factors determine a healthy relationship. However, sex does play a part in a healthy relationship, but it does not. I'd like to comment on the question. There are certain groups of people who don't believe in sex before marriage. And there are some groups who don't believe in sex after marriage. Okay. And I would venture to say to that I can almost hear a sex therapist out there say, well, there are other things that you can do to have a very intimate and enjoyable and pleasurable relationship. And I'm not a sex therapist, but I can hear them saying, Oh, this is off the hook. Let's take another call from home. I call, Are you on City Line live? Um, yes, I have. Um, when I'm able to. Um, maybe to to, to, to get an erection. But. But my record comes so fast that that it, you know, my sex, um, encounter would be over so fast, cause. Cause I can't keep it up. I can't keep it firm and mental. I'm not finished. I want to know with anything I can do to. To keep this process longer. Which direction are you talking about? Which one of the erections that you get. Are you talking about? Oh, well, I get on. You know, when I first started intercourse, it would be like a minute and a half before I can have and I have an erection. And then after that, you know, my penis just goes down and it just can't. And the first one may be rapid, the second longer, and the next one even longer. But a 31 year, you're kind of over the hill sexually. And if you get one, then you get some sleep. Then you can probably get another and try to be rested and you would probably last longer. Okay. Thanks for your question. Quickly, though, we start off the show defining impotency as the inability to either obtain or maintain an erection. Is there any instance where an individual is able to briefly obtain an erection but not maintain it where a prosthetic device is suggested? Yes, there is, because an erection has to be defined in terms of quality. That is, is the penis fully to mass? And not only that, how long does that erection lasts? Let me ask you a question. You know, black men have been stereotypically typecast as in some certain circles as studs. Does this fly in the face of all of that? And are black men who do have the problem? Do they generally seek help and admit to the problem and seek help? Well, I don't think black men are studs. I think that this is somewhat of a myth. When Mayor Schaefer makes his trip to Annapolis later this year to assume the title of Governor Schaefer, you will be elevated to Mayor Burns, Baltimore's first black mayor. Does that hold a special significance for you, sir? Most certainly does. It's a honor and a privilege to be the mayor of a great city like this and certainly has a lot more added to it by being the first black mayor in Baltimore. I haven't just been a person who was elected office without doing something before. I've been in the political business some 40 years. I've made governors, senators, mayors and congressmen all before. It came a day when I decided I was going to do something from the South where I make other people one I could make myself. Mr. Burns, let me jump in here. We've got a lot of territory to cover in a short amount of time. Usually when you have a new regime, a new head, there are changes that are implemented. When you assume the interim mayor's position, are there any changes you'd like to see, any new ideas you'd like to see implemented immediately? Sure. I guess everybody's priority today is education. There's other important things like housing as all important things like jobs. If you come to the waterfront with those three, I think you can solve a lot of the problems that exist and not only in Baltimore City, but all major cities today. Mr. Burns, what's your reaction to the speculation when we talk about Baltimore's first black mayor that the business community in Baltimore City will be the business base will be dwindling? We also got a lot of we got a it seems that we have a lot of people who are looking for things to do in Baltimore. The businesses and the population is beginning to grow. We stopped at a point beginning to grow, and you're going to find that it's going to grow with people coming in to Baltimore City and bringing people with them and creating jobs for people that's already here. Mr. Burns, in the past, you have certainly enjoyed the support, political support of Mayor Schaefer. Will you still be enjoying that support in your mail bid again? There is speculation going around that that support may be dwindling and the support of Mayor Schaefer supporters for your particular campaigns are maybe dwindling. Are there any truth in those rumors? I don't think so. You know, the time and the day and politics dictates what you do. You can speculate.
