So as a result, we only had 40 minutes to do all the reconstructing in both twins. Now we had two neurosurgical teams. I took one of the twins along with one of the chief residents, and Dr. Long, the chairman of the department, took the other one with the other scientist twin. And we started sewing fast and furiously, trying to get the sinuses reconstructed in that 40-minute time. Cardiac surgeons were noticing the configurations, for instance, the configuration that this open sign is here and cutting pericardium, which is the covering of the heart, to those shapes and handing them to us so that we could sew them directly in without taking time to shape them. Does your sinus have an opening in it? The sinus is a big opening. It's a giant channel through which the blood runs. All the blood from the brain drains into the sinus, and that sinus carries it back away, dumping it eventually into the jugular veins, which carries it back to the heart. And it's running through those sinuses, particularly the posterior sinuses, at a very fast rate, to the extent that we're one to place a hole in that sinus. about the size of a pencil, exsanguination would occur very rapidly. You could cause death in a matter of minutes, if not just one minute, from the loss of blood. So at any rate, due to the fact that we didn't have to do any shaping of the sinuses, of the tissue that the cardiac surgeons were giving us, we were able to complete the procedure in just under one minute as the buzzer was going off. We were able to start the pumps back up. So it was a little bit tense. We didn't know that we were gonna make it or not and it obviously required a lot of cooperation on everybody's behalf. Thank you very much. Everybody had to keep a cool head and do the best that they could do, and we just didn't make it under those circumstances. And the blood is now being pumped back in. You can see it coming through the tubes. And you can see that the brain is no longer as pale. And this was the first time the twins were in their own separate beds. They had a lot of complications in the post-operative period. cardiac arrest, lung collapse, kidney failure, problems with the blood, infection, wound breakdown, liver hypertrophy or swelling of the liver, you name it, they had it. And it required a tremendous amount of effort on behalf of a lot of people working around the clock those first two weeks to keep them alive. I'd say it requires... Just about everything known to a modern man to keep them alive during that time. Every time my beeper went off, I said, oh no, what is it now? But eventually we got through that critical period. And I must say that the pediatric intensive care unit here is absolutely incredible to be able to pull off this because I think the post-operative care is probably as spectacular as the surgery. And without that type of post-operative care, they obviously wouldn't have made it. We were very, very pleased with the whole scenario in the long run. And we had a lot of problems with the scalp and the wound breakdown. And it required a lot of very, very advanced techniques in terms of taking vascularized skin and muscle from the back. And, uh... placing that on the skull, and Dr. DeFrain and other plastic surgeons were instrumental in doing that and took them back to the operating room at least 10 times to do various stages of those procedures in order to get those grafts to take. But eventually, they did take. It was a lot of work, but they did take. And although things looked pretty rugged at that stage of the game, it wasn't long before things start healing up.
