Now, one of the things that traditionally happens with people who are immobile and have a tendency to lie in the same position is that they will become flattened. So in the old days, for instance, with hydrocephalus, sometimes when it didn't cause death, The heads would become very large and the patients were relatively immobile and they would simply turn them from side to side and what you would see in these institutions are people with heads that were this big around but were that narrow and they just looked like big pancakes and they would keep turning them over. The same kind of thing is starting to happen here is the back of their heads are flattening because, obviously, being connected the way they were, and they... would not be able to roll or mobilize themselves. So they can only move when somebody picks them up and puts them in a certain position, which is one of the reasons we felt it was necessary to separate them, because the quality of life in an immobile state would not be very good. And it's extended out laterally, covered the entire cerebellum, so both babies. And so there was like a high pressure venous lake covering that entire portion, which we had not anticipated. And that meant that we had to go on to hypothermic arrest, where we would cool the temperatures of the body, stop the heart, pump the blood out prematurely. we had already pretty much designated what we were going to do during the hour of hypothermic arrest that we had available to us. Then we had to factor in this into the hour and it made things a little bit anxiety-producing, I would say. And then here is at the beginning of the expansion of the scalp. We went over to Germany in May of last year. And Dr. DeFrain, one of the plastic surgeons, put in a scalp expander, which could be inflated with saline gradually. Every week, put in about 50 cc's. Until the thing gradually got larger. larger. And here you can see it's starting to become a very substantial mass. And here you can see it's almost as large as their heads. Most of the sessions were for the purpose of making sure that people knew where they were standing, what would be done first, what would be done second, what would be done third, the types of instruments that would be used, and just mental exercises. Because obviously, we couldn't really practice the surgery itself. But we wanted to make sure that all the ancillary procedures and all the support procedures and staffing and instruments were in place, and that there wouldn't be anything that we would need that we didn't have. Those are just teaching slides, you can skip through those. Yeah, you can skip through that. This is just during the anesthesia induction period. And you can see some of the many members of the anesthesia staff who were involved with putting the patients to sleep. Anesthesia and Siamese twins is very complex because although they shared the vascular systems in the head, they of course have separate hearts and separate lungs, separate kidneys. And somehow, one has to make all those things mesh. Everything has to be relatively homeostatic. And it's not. One thing that one easily finds is that if you're inducing anesthesia in Siamese twins, and they're not perfectly level, certain of the chemicals will tend to pool on one as opposed to the other. And that one will have a more dramatic effect from that drug than the other one will. If it's, say, a drug that slows the heart rate, then the one that's dependent will have a slower heart rate than the one that is above. And then that can cause some very significant hemodynamic problems. So the anesthesia is a problem. And it really took three or four hours just to stabilize things from the anesthetic point before we could make the incision. And here you can see the scalp expander, very large, basically as large as the head itself and either of the twins. And everything is prepped and draped out at this point. The operating room nurses actually designed these drapes. They took them home and designed them in such a way that they would maintain sterility, even after we separated them. They had a little plan. They had little accordion sleeves and things that would drop down when the tables moved apart so that everything would remain sterile. We would have to waste time trying to re-sterilize things. because that, of course, would be during the time when we were in hypothermic arrest. We only had one hour of hypothermic arrest time, so every second was precious. And these are the scalp expanders being removed. and just pictures of the opening ceremonies. bone being removed, and the covering of the brain being gradually opened. This one's backward, I don't know if you want to focus on that one. This is the sinus, very large sinus connecting the two brains.
