He laid this off a rap on me about being the number one killer among black males and how it sneaks up on you. That's right. The number one killer. And he says, as of today, I've got to change my life. I got to take these pills every day. I got to change my diet. I got to visit him very often and take tests. I tell you something, man, I don't understand it. And I'm not ready for this. I got 300 miles to feed. And don't make sense. It's just don't make sense, man. We have witnessed a number of. Welcome to City Line. The topic today is very, very important to our own community and we have gathered specialists to talk about it. Our first guest is Dr. Elijah Sanders, who is the president of the International Society on Hypertension in Atlanta. And to sit in on and next to Dr. Sanders is Dr. John Chazelle, who is a consultant. And you'll help me with the second title. Thank you very much for sitting here. And before we begin with them, we want to discuss with you a few questions as we wait. We have a little quiz here for you. Some things about hypertension that you may or may not know that we hope to give answers to before the show is over. But I want to ask yourself these questions and see how well you score. We'll let you know how you do later on. First, the first question is high blood pressure can be reduced by getting regular exercise and decreasing the amount of salt in your diet. Is that true or false? The range of our experience. The second question People only have to worry about high blood pressure when they get old again. True or false? Third question Hypertensive patients should only take their medication when they feel upset. True or false? And finally, people who suffer from high blood pressure often feel sick, have headaches, blurred vision and other symptoms to warn them of impending danger. True or false? We'll give you the answers to those questions a little bit later. First, let's see if we can prevail upon our guest to describe this, this medical problem that we've all heard of before. That is not new in terms of us hearing about it, but seems to be an ongoing problem. What is high blood pressure? IE high blood pressure? And what it is, and I should say what it's not it's not high stress, high emotions, high tension, nervousness and so forth. People think it's that. But it's not that it is a true increase in pressure in the vascular system such that the heart has to pump very hard and it gets enlarged because the pressure is too high and the heart has to have higher pumping pressure and it gets like a muscle of the arm gets larger, the more the heart it works. It also leads to stroke because the brain, the blood vessels in the brain become thickened and damaged and rupture. You get a stroke. It also leads a kidney damage. The kidneys become damaged and of course, you're going to have heart attacks. These are the results of high blood pressure. And what is the what seems to be the most common reason that this remains such a health problem? It's a controllable disease. Controllable problem. Why is it such a problem still? Okay. There really? You have two questions in one. One, of course, is why do we have high blood pressure? What is this all about? We don't know the answer to that. There are several risk associated with it. Being black gives you a greater chance of having high blood pressure. Being overweight gives you a greater chance of having it. Being poor and lower educated as a group tend to have more high blood pressure. Now, the other question is why is the problem of control? There should be no reason for that, but there are reasons for it. There are problems with that in the black community especially. What are some of the things that you advocate to to bring down the pressure and to control it? Well, the first thing to do is to understand that although hypertension is a serious disease, you the person can and should be in control of it. So that means if you're diagnosed as having high blood pressure, the first thing you want to do is to go out and get a blood pressure instrument. They're very easy to use now. They don't require stethoscopes and they're fairly accurate. And by monitoring your blood pressure at home and at work in various situations, you can begin to determine the effect of your food, your job, your relationships on your blood pressure. And it's well known that blood pressure goes up when you in the doctor's office. So the pressure that you have in a doctor's office may not be the average blood pressure that you will carry. So the first step is to understand what the numbers are and what they mean. And then as you monitor that, you can see the effects of switching to a healthier diet and of exercising and of finding some way to balance out stress, to keep it from becoming distress. You can determine the effect of all those. And as you see these numbers go down, you know that you are on the right path. I don't think there is enough emphasis on that. And you can help your doctor reduce your medication as he sees and you see your blood pressure coming under control. That's right. Let me ask you, you're not a medical professional. How did you get into the practice of at your establishment actually offering the blood pressure test and why? He's suggested me. Well, you asked me whether what I would be interested in, and I told him I didn't know, but so he said, Dr. Kong is talk to me about it. When Dr. Kong came to talk to me and I felt it was very necessary need for the body. So I just took a shot of him. What what kind of response did you get from your clientele when you announced? Starting. At a certain point, we're going to place an emphasis on high blood pressure testing and this facility, Harry's Afrojack. Well, some of that, they laughed at me at first, but we start talking to them more or less when they get in a chair and place a lot of effort on the fact that is a daily killer. We didn't know that much about it. So and I found a lot of people did not want to know about them having a problem. So I just kept after them and talked my people into talking to. And so we eventually got around to them. They decided to try it and I just more or less encourage them to take a chance at it. How many people do you think you've involved and been involved in tested since you started? Well, there's quite a few hundred so far, but I can't say I haven't have the high blood pressure. While we have a high elevation, we recommend urban research. You know, let them do the follow up on it. So that's what we do there. Okay. We're going to continue our discussion in just a minute. Yeah, that's one of the new things with my dad. You don't know anything about my wife's fried chicken? Yeah, man. Break his back all day. He's entitled to the things. You know what I mean? Well, how can I taste it with no solid coated man? Oh, come on, Jesse. You know, we got a little enjoyment out of that, but that is not an uncommon reaction to, I would suspect, to someone who is first discovering the need for this. Tell us a little bit about the interaction of diet and hypertension. Yeah, I think probably not to change our topic and say a lot about the role of diet. Well, to begin with, that probably is the one thing that everyone can take charge of. If you look at all of the factors that contribute to the development of hypertension, the one thing that you can take charge of today is what goes in your mouth and which as well. So if you choose the right foods and you have this high blood pressure instrument, you can watch your numbers go down and you can convince your doctor that you are in control of it, because the doctor wisely insist that you take your medication until he or she is convinced that you are in control of this hypertension. But if you eliminate processed foods like baloney, padded meats, hot dogs, sausage, bacon, those sorts of things increase the amount of fruits and vegetables that you eat, then you can get control of it. There's been an underlying emphasis on the role of diet and hypertension because many physicians have not believed that you can control what they call serious or malignant or accelerated hypertension with diet. And the reasoning was, is that most they don't think most people will be diligent enough to eliminate all of the things that elevate your blood pressure. So for the most part of diets in the black community are generally high in what fats and fats and high in sodium as they are processed. Meats like bacon, ham, baloney have three kinds of sodium. They are salt, which is sodium chloride. They have monosodium glutamate, which is a meat tenderizer, because the meat is usually of poor quality and it has sodium nitrite so that the, uh, the person who is selling the meat can keep the meat long enough for you to come in. So we've got to get rid of the fat back in the grooves and the beans and the black coffee and other seasonings that can make it taste just as good. Okay. Now, that's another issue that also mean only because we kind of love salt and pepper. Does that also mean applied to pepper? No. Okay. But you you do have to read labels because salt is hidden in so many things. Sodium is in and see so many preservatives are used like the delicatessen cuts. You know, people will go out and they will buy a turkey roll or chicken roll, but it's processed with this monosodium glutamate and the sodium nitrate, even if it doesn't have a. So how do we get people to seek and follow treatment for it for high blood pressure? How do we develop develop an interest in that whole person? There's a lot of motivation involved there. You have to constantly reinforce what the physician tells the patient to do. You never know. So we want to try and keep close tabs on a patient so we can continue to motive motivation. Part of the treatment is motivation. Uh, excuse me, but is motivation a key issue now in trying to get people to seek and maintain the treatment? Yes. What type of motivation do you use? Depends on the motivation. Depends on the person. You know, it's like reinforcing a child. Everyone's different. So we have to ask as far I agree with Dr. Tisdale about, for instance, blood pressures being different in the home and at the doctor's. I would do blood pressures in a person living room if they went to the doctors a couple hours late. It's kind of sky high. So they're all kind of motivational techniques. But it's important to develop a relationship with their patients. Doctor also. We don't send our double messages to patients. We called the high blood pressure the silent killer. And that's because so many people feel okay. They feel well, don't they? And yet they don't know. Yeah, that's a very important concept. And you wonder how there could be a disease that has so common kill so many people, and yet only about 25% of the at least the males control maybe about 40 to 50% of females. They have a better control rate. But still, there's a lot of that uncontrolled 75% for males and 50% more or less for females. And it's because of several things. The principal one is the lack of symptoms. Let me interrupt you. So while the silent killer is at work and people are unaware of it and feeling pretty good, what's happening in the body? Okay, that's that's why a silent killer at the same time, while you're feeling well and having no symptoms at all, you are having increased hardening of the arteries of the heart that could lead to a heart attack within a few years. You're having increasing enlargement the heart while not feeling it, that eventually lead to heart failure. Your kidneys are being damaged because of this high pressure and the decrease in blood flow to the kidney. That's all occurring. And then when you feel it, it's a little late and you boom, you have a stroke or heart attack or heart failure. So people have to understand that it's hard said to be motivated to find out about whether they have high blood pressure early and by all means to follow through with the advice, whatever the doctor gives them. Let me ask you a give me a typical scenario. A customer comes into your shop. Do you offer the service or do they actually come there seeking that particular part of your your service? Well, basically, they don't come seeking it initially. But what we do know, what do we need? And we have a little of what you showed on the monitor. We give them a quiz and let them see what they know about. That's the first thing we know and say this is like a joking thing. So what do you know about hypertension? Some of those people say, well, I don't know. The you tell them whether or not they have a high elevation or what what they expect. Then you say to them, Now, if you do have a high elevation, I suggest you see your doctor, so forth and so on. I like the idea of it being done in barbershops and in and then the introduction of it in churches as well, because of the relationship that's established, which gives confidence. We'll get into that a little bit later right after this break. Hey, doc, listen, I hate to admit it after I caused everybody so much grief, but I do feel better. I don't know if it's the weight I lost or the diet I'm on or whatever it is. I get up in the morning, I'm ready to go. And Theresa, she keeps me straight, too. I don't know what you told her, but it should work. She watches me like a hawk. She cooks the food, right? She makes me take that pill daily. Listen, I feel good, man. Hey, you help me out, man. People of all ages get high blood pressure under 50. It is more common in men. Over 50 women are often more affected. The answer to the third question is false. The medicine you take for your blood pressure will not calm you or make you feel better Take prescriptions as prescribed. Finally, the fourth question is also false. No one can tell how high blood, how high blood pressure is by how they feel. That is why you need to get it measured frequently. I just want to talk about the one question having to do with medication. Once you start medication is that you have to take that for the remainder of your lives. Well, we have modified that a little bit. For the most part. We tell people that it's a life long experience since more often than not, they will need to stay on it. There are some exceptions, some people who can change their lifestyle style, as Dr. James said, who can reduce their salt significantly, properly exercise, reduce stress. Some of them may be able to go off medication, especially if their blood pressure was not very severely elevated. But I would say that's more the exception to the rule possibly, or I take exception to that, because if we stop and look at the laws of nature and a little common sense, we don't get high blood pressure because the creator sent us here without the medication. Hypertension is a gift from your creator to teach you to change your lifestyle if you eat. But it's true. But the three questions and in terms of the vignettes that black males particularly should ask themselves and teach their children, task number one is what kind of energy is the food or drink I'm about to take in going to do for me? Going to give me number two, what is this food going to do for my body? Temple, this temple of my mind and my soul? And number three, if I have a disease like hypertension, what is this food or drink going to do to my blood pressure? If you teach your children to ask those questions before they select the food, then they will select the wrong foods. Because children, like all the other creatures in nature, follow examples you teach by example. We're dealing with a generation of black men who have and who were not brought up to understand or ask themselves those questions. Briefly, how difficult and how will we convince them to make these lifestyle changes and to follow up on the information that we're giving would be very difficult, but it's really worth the effort. Okay, I worked a job in pediatrics. That happens. And I believe that like with most problem diseases and catch things early in life, it may have a better prognosis. Dr. Silas has the theories right and that it yourselves is right. I think we have to start. A lot of times have children. In Baltimore, we are now convening for these three days the third International Interdisciplinary Conference on Hypertension and blacks, emphasizing that the problem is not only one of this country of blacks, where we've done most studies, but it seems to be also a worldwide problem, largely among black people, but other people around the world, especially in developing societies. So we've had we have in town people from Africa, from South America, from the Caribbean, from Europe, and of course from this country, researchers, doctors, clinicians and etc.. On a third day which is today, we will be having a health fair and a major screening education, somewhat social activity. We have in about a way to place it. We're having it in Baltimore at the fifth Regiment Armory from 12 until six. We also having it at a high school in Washington, D.C., because we're having the same program this morning. We would have sent our delegates, these worldwide experts out to churches to get a message from the pulpit on high blood pressure. Okay. So and the health there is free health fairs, free blood pressure, check. You can get your cholesterol checked, which is also important. But we had another time on the program and you can get some messages on high blood pressure and you can get a chicken dinner, but. That'll be all for you. Okay. There is never enough time to talk about those issues that are important to us.
